Instructions for downloading forms
- Double click on the link at right for the form you wish to download.
- The form will open automatically in your browser using Adobe Acrobat Reader.
- You must then save the form to your hard drive. To save the form, select file, save as from the Acrobat menu (top left corner).
- Save the file to a folder on your hard drive. Remember the folder name so that you can open the file later.
Instructions for completing and returning the form
- Open the file using Adobe Acrobat Reader.
- Complete the form by typing in the information or selecting the correct check boxes.You will know when to type the information because the cursor will become an "I" bar. Right click and type.
- When you have completed the form, select file, save as and save the completed form to your harddrive.
- You can then send the form back to our office as an attachment.
